    Please read the following declaration statement and check the box prior to submission.

    • I confirm to the best of my knowledge and belief that the information and documentation that I have given in my application for registration is true, complete and accurate.

    • I understand that if any part of my application is found to be fraudulent or dishonest (either by act or omission), I may be subject to Fitness to Teach proceedings meaning that my registration may be refused or removed and/or I may also be reported to the police.

    • I understand and consent to GTC Scotland seeking to verify or gather information provided by or in relation to me in connection with my application for registration and otherwise in relation to my ongoing Fitness to Teach and suitability for continued registration with GTC Scotland. I understand and consent to GTC Scotland contacting any person or body who may hold such relevant information in relation to me. I understand that this may include my current or previous employers, Police Scotland, Scottish Courts and Tribunal Service, Crown Office and Procurator Fiscal Service (or the equivalent body in any other jurisdiction, as may be required), Disclosure Scotland and any other regulatory body, the UK or any other country, with which I am or have previously been registered. Where such a person or body is approached for information, I consent to them providing GTC Scotland with any information requested.

    • I agree to my application to join the Protecting Vulnerable Groups Scheme (PVG Scheme) being submitted by GTC Scotland on my behalf.

    • I understand that I must remain a member of the PVG scheme as a requirement of my registration with GTC Scotland. As a consequence, I understand that from time to time I may be requested by GTC Scotland to provide a PVG Scheme record. I agree to consent to GTC Scotland seeking an updated PVG Scheme record upon such a request.

    • I confirm that I have read GTC Scotland’s privacy policy and understand that GTC Scotland may process my personal information in the ways set out in that policy.

    • I declare that if my application for registration is successful, I will comply with all applicable rules, codes and standards that GTC Scotland may set from time to time, including those relating to my conduct and professional competence.

    • I understand that I may not be employed as a teacher in a school in Scotland (or take up a place on the Teacher Induction Scheme) unless and until my application has been successfully processed and my application for provisional/full registration granted (where applicable).
